Helotes, TX — April 1, 2025, Cody Bell sustained fatal injuries due to a work accident that took place off Bandera Road.

OSHA release a preliminary incident report summary about the accident. It happened northwest of San Antonio off the corner of Bandera Road and Loop 1604.

The factors surrounding the death of 35-year-old Cody Bell are unclear at this time. Authorities did not report a confirmed cause for the accident, and it’s unclear what work Cody Bell was doing at the time.

OSHA listed the case status as “closed.”

It might confuse people to see an OSHA summary give absolutely no information and then be considered “closed.” Maybe there’s more information behind the scenes, and their focus was on getting answers for the victim’s loved ones rather than the public. However, in my experience, it’s very common to see OSHA leave a serious accident without any clear answers months or years following the incident.

At the end of the day, OSHA has their priorities. Those don’t always align with getting a worker’s loved ones the help or the answers they need. As such, navigating Texas work fatality law—the most complex work fatality laws in the whole country—can require efforts from independent professionals. They can put the needs of the family first and ensure there’s a proper resolution, even if OSHA decides they have higher priorities to focus on.
