David Martinez Killed in Forklift Accident in Laredo, TX
Laredo, TX — November 12, 2021, David Martinez sustained fatal injuries as the result of a work accident involving a forklift.
Investigators said that the incident took place at around 11:45 a.m. at 5002 Riverside Drive. This appears to be the address for Transport Services Inc., though the company mentioned in the public Occupational Safety and Health Administration investigation summary is Gogo Forklift Service, Inc.
According to OSHA, 50-year-old David Martinez was approaching a forklift that was in operation by another person. While approaching, investigators believe the operator did not see Martinez, and the forklift ran over Martinez. This resulted in fatal injuries.
Currently, the OSHA summary is not listed as "closed." It shows potentially one violation and lists a possible penalty of $2610. Additional details surrounding the fatal accident are unavailable at this time.
