Commentary by Attorney Michael Grossman
Perhaps this was all just a terrible, unavoidable mistake. People tend to think that if there were mistakes here, OSHA would call it out. Unfortunately, that's just not how things always go. Sometimes, things slip through the cracks, details go overlooked, and companies that should face consequences for getting someone killed escape responsibility. Maybe that isn't true here, but are there any other details to shed light on? Did other departments look into this? Did the victim's loved ones get better answers than the unacceptable scarce details here?
