Juan Perez Ortiz Killed in Work Accident in Bacliff, TX
Bacliff, TX — August 25, 2022, Juan Perez Ortiz was killed as the result of a workplace accident at around 5:30 p.m. along Boulevard Street.
Occupational Safety and Health Administration statements were given in an incident summary released to the public. The case status of this report is currently listed as "Open." It appears the incident happened at 4424 Boulevard Street, which appears to be between Avenue A and Avenue B.

According to OSHA investigators, 34-year-old Juan Perez Ortiz was doing HVAC repair work at the location. While doing so, investigators believe that Perez Ortiz was electrically shocked. This resulted in fatal injuries due to electrocution, officials said.
Right now, the exact cause of the accident has not been listed by OSHA in public statements. The incident summary does currently list four alleged safety violations, all of which were labeled as "serious." A recommended penalty of $4, 688 is shown in the summary at this time. Currently, the status of the investigations and a conclusion of what exactly caused the fatal incident remains unconfirmed.
