A lack of signage or signals? Does this area see a high rate of pedestrian accidents? Was alcohol a factor? Did any nearby video cameras capture the incident on video? These may seem like standard questions to ask, but it would shock people how often we have independent investigators look into an incident, and authorities never bothered addressing these concerns. Perhaps they were more thorough here, but that remains to be seen.
