David Delacruz Killed in Work Accident in Houston, TX
Houston, TX — June 6, 2022, David Delacruz was killed following a work accident at 10:47 p.m. alone Pine Vista Lane in Houston.
Officials said that the incident happened at 6650 block of Pine Vista Lane, which Google Maps shows is a facility for the company listed in the OSHA investigation, Labatt Institutional Supply Company. The business appears to operate as Labatt Food Service, a food supply company.

OSHA investigators said in their public statement that 27-year-old David Delacruz was moving through a doorway with a roll-up door. Somehow, that door hit Delacruz, resulting in fatal injuries. No mention of other injuries was made. OSHA listed an alleged violation of their standard 1910.178(n)(4), which states as follows:
The driver shall be required to slow down and sound the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing.
