Commentary
People unfortunately overlook accidents like this all the time. Work accidents don't always get a lot of attention, but a worker's loved ones deserve to know they're going to get the full story. Whether the accident was entirely work-related, some freak occurrence, or even the result of equipment failure due to faulty equipment, it takes thorough investigations to make sure nothing important slips through the cracks. If officials don't take the time to dig into those details, then it wouldn't surprise me if this situation turned out like countless others I've experienced where it took independent investigations to finally bring the truth to light.
