1 Killed, 1 Injured in Truck Accident on S.H. 1 in Halawa, HI
UPDATE (March 7, 2025): Recent reports have been released which relay the fact that the woman who was injured in this accident ultimately passed from her injuries on March 5, 2025. Additional details are not currently available. Investigations continue.
Halawa, HI — December 31, 2024, two people were injured following a truck accident shortly before 2:45 p.m. along State Highway 1.
According to authorities, a passenger van occupied by a 73-year-old woman was traveling westbound on S.H. 1 in the vicinity of the Aiea exit ramp when the accident took place.

A box truck had apparently been traveling in front of the passenger van. Officials indicate that, for as yet unknown reasons, the rear door of the box truck opened. Due to that fact, the truck came to a stop in order to again close its door. However, it was reportedly rear-ended by the passenger van.
The 73-year-old woman who had been behind the wheel of the van was entrapped in the wreckage, according to reports, and had to be extricated by emergency personnel. Once freed from the aftermath, she was transported to a local medical facility by EMS in order to receive immediate treatment for the critical injuries she incurred over the course of the wreck.
The man who had been behind the wheel of the truck was also seriously hurt and had to be taken to a medical facility for treatment. Additional details pertaining to this incident—including the identities of the victims—are not available at this point in time. The investigation is currently ongoing.
Commentary
When accidents like this occur, people often focus on the immediate sequence of events, such as the van rear-ending the box truck. However, with over 30 years of experience handling commercial vehicle cases, I know these situations typically involve deeper questions about responsibility and preventable failures that need thorough investigation.
One of the most pressing questions here is what caused the rear door of the box truck to open in the first place. Was it a mechanical failure, or did someone fail to properly secure the door before the truck began its route? If the latter is true, who was responsible for ensuring that the truck was safely loaded and secured? Such an oversight would raise serious concerns about whether standard safety protocols were followed, potentially contributing to the crash.
Another important area to investigate is the condition of the box truck itself. Regular maintenance and inspections are essential to ensure that vehicles on the road are in safe operating condition. Was the truck up to code, and were its components—including the rear door mechanism—working as they should have? If not, a mechanical issue might have been preventable with proper care and attention.
It’s also crucial to examine the trucking company’s role in this incident. Companies have an obligation to ensure their vehicles are roadworthy and their drivers are adequately trained. Did the company maintain the truck properly? Were the driver’s training and supervision sufficient to handle the situation? Was there any pressure placed on the driver to meet a schedule, potentially compromising their judgment when the truck needed to stop? Investigating these questions can reveal whether broader practices within the company contributed to what happened.
Ultimately, the goal is to uncover the truth and provide answers to those affected by the crash. Even if it requires conducting an independent investigation to obtain critical evidence, it’s essential to examine every factor to ensure that all impacted by the incident receive the clarity and closure they deserve.
