People might be quick to just write this off as a freak occurrence. Hopefully, authorities are being more attentive to the facts. This could involve anything from a simple mistake to a serious vehicle defect, and it's important to preserve evidence to find out one way or another. If authorities can't show they're doing that, then there may need to be more thorough independent investigations.
